Omalizumab and apremilast

Question:

I have a 35 year-old male patient who has a history of psoriasis and chronic spontaneous urticaria. He is not responding to Allegra 180 mg daily plus Pepcid 20 mg twice per day plus hydroxyzine 25 mg – one to two tablets at bedtime. He is also on Otezla for his psoriasis. Is it okay to start him on Xolair given that he is on Otezla. Would there be any interactions between these two medications?

Answer:

There is very limited data on combination of mAb and apremilast. A search of PubMed and review of the package insert did not reveal any studies using these drugs together to any drug interactions. If the decision is to move forward with omalizumab use in this patient then a thorough discussion of the lack of data and potential for untoward side effects should be undertaken and documented.
