Omeprazole reaction

Question:

A 59-year-old male developed swollen uvula while on omeprazole, he was not taking any other medication at that time, prescribed or OTC. His swelling resolved with Claritin. His GERD is not controlled with Famotidine 40 mg daily, recent endoscope was negative to H.pylori and negative for EOE. His gastroenterologist wants to put him on PPI, what is the best way to help this patient.

Answer:

I always check a C4 on patient with Angioedema as rarely I have uncovered cases of acquired angioedema. I would refer you to the JACI IP Practical Guideline for the Evaluation and Management of Drug Hypersensitivity. This is a section on PPI testing and if needed desensitization.
