Dupixent dose

Question:

What is optimal duration of gap after which we might have to consider Dupixent loading dose again for eczema or asthma indication. I have a patient who had received last dose in April 2022 and returning in August 2022 for resuming Dupixent for eczema

Answer:

The package insert for missed doses for Dupixent states:
"2.7 Missed Doses
If a weekly dose is missed, administer the dose as soon as possible, and start a new
weekly schedule from the date of the last administered dose.
If an every other week dose is missed, administer the injection within 7 days from the missed dose and then resume the patient’s original schedule. If the missed dose is not administered within 7 days, wait until the next dose on the original schedule.
If an every 4 week dose is missed, administer the injection within 7 days from the missed dose and then resume the patient’s original schedule. If the missed dose is not administered within 7 days, administer the dose, starting a new schedule based on this date."

There was no recommendations for re using the loading doses if several months of dosing have been missed. I think this would need to be a clinical decision to be made based upon how the patient is doing. For example, if the eczema is not under control since missing months of the Dupixentthen one could consider a this as a "new" start and give a loading dose. Conversely is the patient is under decent control then continuing the current dose may be adequate.
