Oral pea food challenge

Question:

3/19/2019
Our office has been doing food challenge for many years. We are planning a pea challenge which I have not done in the past. Do you have any guidelines for this? Also are there any changes or updates to doing a soy challenge. I've read about soy flour and also using soy milk. Any information regarding these challenges would be greatly appreciated.

One more question please, any information about palm oil? Several of our patients have voiced concerns about needing to avoid palm oil with other food allergies. Anything I have read suggests palm oil is not a concern.

Answer:

I referred this question to one of our food allergy experts.

Here is his response.

I strongly suggest using this reference: Nowak-Wegrzyn A1, Assa'ad AH, Bahna SL, Bock SA, Sicherer SH, Teuber SS; Adverse Reactions to Food Committee of American Academy of Allergy, Asthma & Immunology. Work Group report: Oral food challenge testing. J Allergy Clin Immunol. 2009 Jun;123(6 Suppl):S365-83. doi: 10.1016/j.jaci.2009.03.042.

Table 5 in the reference describes amount for beans and soy and the text discusses open food challenge dosing. An updated report about food challenges should be out soon, so keep an eye out for that as well.

Regarding Palm oil, I am not aware of any literature to alter your conclusion.

Sincerely,
Scott Sicherer, MD FAAAAI

I hope this information is of help to you and your patient.
