Horse IT

Question:

I have a patient who rides horses and has recently developed an allergy to his horse. He has requested starting IT. I have seen papers with limited number of patients. Are there any recommendations? What is effective dose?

Answer:

There is very limited data on horse IT. This article (https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6934807/) discussed the development of hypo allergenic variants of the major horse allergen Equ c 1 which in of interest but does not address the specific question raised. In my practice, we have several patients in horse AIT because of either professional or personal exposure. We typically target a final concentration of 1:250 w/v horse in the red vial (most concentrated AIT vial) and adjusted as the patient tolerates and symptoms dictate.
