Minor determinant mixture for testing for penicillin allergy

11/4/2009
Question How can one obtain the minor determinants of penicillin for testing? With Pre-pen coming back on the market and PCN G shown to not be an effective testing agent, we need to use the minor determinants.
Answer Thank you for your recent inquiry.

Unfortunately there is no commercially available preparation of minor determinant mixture, and therefore, should you wish to use it, at this time you would have to make your own. It is my understanding that commercial interests are working on manufacturing a minor determinant mixture, but at this time it is not available.

Should you wish to prepare your own minor determinant mixture, the following reference will give you instructions in this regard:

Macy E, et al. Skin testing with penicilloate and penilloate prepared by an improved method: Amoxicillin oral challenge in patients with negative skin test responses to penicillin reagents.
Journal of Allergy Clinical Immunology 1997; 100(5):586-591.

Thank you again for your inquiry and we hope this response is helpful to you.

Sincerely,
Phil Lieberman, M.D.
