    YES   NO

  2. Has your asthma stopped you from participating in activities such as exercise?
    YES   NO

  3. Do you have shortness of breath often?
    YES   NO

  4. Have your asthma symptoms (wheezing, coughing, shortness of breath, chest tightness or pain) woke you up at night, or earlier than usual in the morning?
    YES   NO

  5. Have you used your rescue inhaler or nebulizer medication (such as albuterol) more then twice a week?
    YES   NO

Results

If you answered yes to one or more of the questions, your asthma may not be as well controlled, as it should be. Consult your physician as soon as you can. Only he or she can determine whether you need to change your treatment plan.

If you answered no to all the questions, your asthma seems to be adequately controlled. However, it is always smart to see your physician regularly. That is always your best resource to make sure your asthma is under control.
