Ethical judgment & practice
      
According to Pellegrino and Relman*, “Medicine is, in essence, a moral enterprise.” The demonstration of ethically-sound judgment, practice, and behavior is listed as one of the components of professionalism in virtually every definition of the competency.

The resources included in this section of the module are designed to provide learners with information on the concepts of ethics and professionalism in general, as well as information concerning specific topics such as informed consent and patient confidentiality.

American Medical Association
(AMA)
The AMA website provides a wealth of information relevant to the competency of professionalism. Training programs may find multiple uses for these materials ranging from formal presentations or symposia to informal discussion.

Extensive ethics resources are available. The Ethics Resource Center provides materials such as the Virtual Mentor, the Ethics Forum column in the American Medical News, and Professing Medicine, a special issue of the Virtual Mentor dedicated to issues of professionalism,

The Institute for Ethics provides information about topics such as the Ethical Force program. The goals of this program are: “(1) to identify and promote shared ethical expectations for all participants in health care, (2) to develop valid and reliable measures of achievement of ethical expectations, and (3) to encourage the widespread adoption and use of these expectations and measures.”


One section contains general legal information related to a number of issues for physicians. Issues relevant to professionalism include:

     physician-patient relationship issues, consisting of information about:
          • patient confidentiality
          • ending the patient-physician relationship
          • informed consent

      compliance issues, consisting of information about:
          • health care fraud and abuse
          • professional courtesy
          • Americans with Disabilities Act and hearing interpreters
          • a Compliance Interactive Tutorial that provides lessons and references related to fraud and abuse and to compliance. This resource is available only to AMA members.

     business and management issues, including information about:
          • the National Practitioner Data Bank
          • alternative dispute resolution


Another section of the website, titled ethical opinions/guidelines, contains information on gifts to physicians from industry. Very specific questions are addressed in a frequently asked questions section.


Another section provides information on current ethical opinions on a variety of topics.

Additionally, the site provides an ethics module that can be worked through for CME credit.

American Board
of Pediatrics
(ABP)
The American Board of Pediatrics has developed materials that can be useful for training programs for teaching and assessing professionalism.

Their statement of the components of professionalism can be used for discussion. A series of vignettes, portraying ethical issues, can be used by groups.

Ethics in Medicine The University of Washington’s Ethics in Medicine site was developed to promote, expand, and integrate bioethics education in medical education. It provides information on major principles of medical ethics, and information and brief interactive cases on 35+ bioethics topics including:

     • professionalism.
     • confidentiality
     • ethics committees
     • end-of-life issues

Several bioethics tools are available for use. These include a paradigm that considers multiple decision-making contexts and a sample case analysis.

While these materials were written primarily for use in undergraduate medical education, they may be useful across the educational spectrum.

American Board
of Internal Medicine
(ABIM)
The ABIM provides extensive information about the competency of professionalism. Project Professionalism was a project conducted in the early 1990s. Its goals were to:
     • “define professionalism
     • raise the concept of professionalism in the consciousness of all within internal medicine
     • provide a means for program directors to inculcate the concepts of professionalism within their training programs
     • develop strategies for assessing the professionalism of residents and subspecialty fellows during training.”

The resultant resources, available for program use as a downloadable PDF document, include:

     • introductions to topics including abuse of power, arrogance, greed, misrepresentation, impairment, lack of conscientiousness, and conflicts of interest
     • a number of vignettes that can be used for self-study and discussion, ranging from simple one paragraph scenarios to complex cases
     • strategies for evaluation
     • extensive annotated and general bibliographies

Center for Ethics & Professionalism The Center for Ethics and Professionalism, sponsored by the American College of Physicians and the American Society of Internal Medicine (ACP-ASIM) is “devoted to policy development and implementation on issues related to medical ethics and professionalism .” The Center’s website provides numerous resources related to professionalism, including information on managed care ethics, a number of case studies that can be used for discussion, and online access to the ACP Ethics Manual.

MedEthEx MedEthEx is a set of interactive multimedia exercises in medical ethics and communications skill created by the MCP Hahnemann School of Medicine. The site currently provides four cases representing a variety of conditions and patient situations. The exercises allow the user to test her or his skills and to access resources from experts in areas such as communication skills, ethics, and legal issues.

Although these exercises were designed primarily for use at the undergraduate medical education level, fellowship training programs may find them useful.

Society for Academic Emergency Medicine The Society for Academic Emergency Medicine has prepared a series of case vignettes dealing with issues of professionalism. While these cases were developed for use in emergency medicine, many of them are generalizable across specialties.
