2008 Associates (Spouses and Friends) to the American Academy of Allergy, Asthma and Immunology Award of Excellence Scholarship

The Awards

The Associates (Spouses and Friends) to the American Academy of Allergy, Asthma and Immunology will present:

  • $1,000 Award of Excellence Scholarships
  • $100 Merit Awards

Each scholarship is a one-time grant. Scholarship monies may only be used for or applied to tuition, books, and fees for a bona fide course of instruction at an accredited two-year or four-year college or university or accredited technical or vocational college or school. Scholarships must be used within three years of notification to the winner of the scholarship award.

If the winner is unable to use the entire scholarship for any reason, all remaining money will be retained by the Associates for future scholarships at the discretion of the organization.

Eligibility

All high school seniors with documented asthma who will graduate in 2008 and who have achieved excellence in a wide variety of areas including academics, extra curricular activities and community service are eligible. In addition, students must also be United States or Canadian citizens. Children of AAAAI members, employees and contractors are not eligible to participate. Only applications in English will be accepted.

How to Apply:

Initial Application:(Note: The 2008 Application process is now closed)
The student applicant must complete the initial application online. Paper copies of this portion of the application will NOT be accepted. (Cut and Paste can be used to completed certain portions of the required form.)

Secondary Documentation:
The second portion of the application process which includes information from and signatures by the student's physician and high school counselor or principal must be completed and postmarked no later than January 11, 2008 to be accepted. (A parent or guardian must also sign if the student is under the age of 18 at the time of application.) Incomplete applications and those received after the deadline will NOT be considered. This document page will be available in a downloadable (.pdf) format once the initial online application has been completed and submitted. Please note that the physician signature must be handwritten (no stamps or facsimile signatures) by either a Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO). Signatures by a Physician Extender (Nurse Practitioner, Physician Assistant) will not be accepted. There will be no exceptions to this requirement.

Notification of Acceptance:

All completed applications will be evaluated and winners selected by a committee appointed by the Associates. Incomplete applications and those with information postmarked after the deadline will not be accepted and evaluated. There will be no exceptions to this rule. Winners will be notified on or before May 1, 2008. Please note that all selection committee decisions are final. Information about the committee deliberations, the selection process and the scholarship winners will remain confidential.

Winners' names will also be listed on the AAAAI Web site (www.aaaai.org) on or before May 1, 2008.
