Disclosure of Relevant Commercial Interests
Pursuant to the Code of Ethics for the American Academy of Allergy, Asthma and Immunology (AAAAI) and the Standards for Commercial Support of Continuing Medical Education of the Accreditation Council for Continuing Medical Education, the AAAAI requires disclosure of certain information from faculty members of educational activities designated for CME credit by the AAAAI. To satisfy these requirements we ask that all speakers complete the following two forms:The Online Disclosure Management System (ODMS) to supply the necessary information about any commercial interests.
The Faculty and Speaker Attestation Form (PDF), which will be used to resolve any potential conflicts of interest. Please fax completed form to Lori Rathje at (414) 272-6070.
All speakers must complete each form once, regardless of the number of sessions in which they are presenting at the Annual Meeting.
For questions related to the disclosure process, please contact Lori Rathje at (414) 272-6071 or lrathje@aaaai.org.
