Response from Dr. Robert Hamilton:
There are two levels of molecular based allergy diagnostics performed in the world involving singleplex and multiplex systems. In terms of the multiplex system such as the ISAC from Thermofisher Scientific/Phadia is the only system available. In the USA, only 3 laboratories have this technology: Johns Hopkins University DACI-Baltimore; Mt Sinai Medical Center-New York and the company Thermofisher Scientific - Michigan through their PEARL Laboratory. Of these, JHU and Mt Sinai are both research focused and the PEARL Laboratory performs fee for service testing. Remember that the multiplex ISAC measures IgEantibody to 112 individual allergenic components in triplicate with 30 microliters of serum.
 
The singleplex system where each allergen specificity is measured separately is performed in over 150 laboratories in the USA. Anywhere where ImmunoCAP(Thermofisher Scientific/Phadia) or Immulite (Siemens) systems are used, they can measure IgE antibodies to native or recombinant allergenic components to assess molecular specificity of the IgE immune response. You will find that the peanut series (Ara h 1, 2, 3, 8 and 9) and the hazelnut series (Cor a 1, 9 and 14) are the principal allergen specificities I see requested for the work up of allergy patients. The others have more limited clinical utility and are rarely ordered to evaluate patients.
