Response from Dr. Harold Nelson:
The best way to measure and record the results of skin testing is with the longest diameter and the greatest diameter at a right angle to the longest diameter.  For example a prick skin test wheal could be 9 X 4 mm. If it is desired to record both wheal and flare it may be less cumbersome, although somewhat less accurate, to just record the longest diameter of each, for example 4 / 16 mm.
