Sarcoidosis is not a contraindication to allergen immunotherapy nor should it alter your approach to treatment in this patient. The presence of sarcoidosis does not alter the standard approach to asthma therapy. Therefore I would consider treating this patient as you would an identical patient who did not have sarcoidosis.
As far as immunotherapy in particular, as noted, sarcoidosis is not a contraindication to this form of treatment. However, there are no studies that have addressed the issue of the response to immunotherapy in a patient who has sarcoidosis. Therefore there is no way to assess whether or not sarcoidosis would alter the patient's response to allergy injections.
In regards to Xolair, you would use the same indications for Xolair in this patient as you would in any asthmatic.
