JUNE 2003 QUIZ OF THE MONTH . . .
Case Description
You have been consulted concerning a possible drug allergic reaction as a cause of an acute bullous skin reaction diagnosed as erythema multiforme major (EMM). Which of the statements listed below is most appropriate.
- Allergic reactions to drugs such as sulfonamides are common causes of EMM.
- EMM is commonly associated with reactions to herpes infections.
- Since the history strongly suggests a recent herpes infection, this bullous reaction is more likely a manifestation of the Stevens-Johnson Syndrome/Toxic Epidermal Necrolysis (SJS/TEN).
- Since one cannot be sure whether one is dealing with EMM or SJS/TEN, a therapeutic trial of I.V. immunoglobulin (Ig) is indicated.
