Case Description
You have been consulted concerning a possible drug allergic reaction as a cause of an acute bullous skin reaction diagnosed as erythema multiforme major (EMM). Which of the statements listed below is most appropriate.

  1. Allergic reactions to drugs such as sulfonamides are common causes of EMM.
  2. EMM is commonly associated with reactions to herpes infections.
  3. Since the history strongly suggests a recent herpes infection, this bullous reaction is more likely a manifestation of the Stevens-Johnson Syndrome/Toxic Epidermal Necrolysis (SJS/TEN).
  4. Since one cannot be sure whether one is dealing with EMM or SJS/TEN, a therapeutic trial of I.V. immunoglobulin (Ig) is indicated.
